I think your oversized boiler is the biggest problem. I don't see how you could need any more then 200K BTU's. So you're oversized by a third! I doubt the boiler ever gets to high fire. That 0012 can do 50GPM. It was picked because it has a 2" flange for the pipes. I guess all the secondary zones are 3/4", the Phase III uses 1". I would reduce the primary pump down to an 0010 pump and follow the pri/sec piping diagram on the Efficency+. When only one or two zones are calling, the oversizing issue is even worse.
With such an oversizing issue, I wonder if trying to do outdoor reset will only make things worse.
