I'll agree that the tank is probably stratifying due to the low setting of 125F.

Legionella is killed at temperatures above 155F, so I'd move the setting up there. Otherwise, Legionella grows in water at the lower part of the tank below 112F to make respiratory problems in a shower. Running the water that is cold out of the tank to start it up is a bit late.

Although many would like to be 'green' and save money, there is no savings in the hospital. A tempering valve after the tank to mix hot with cold fresh water to a desired temperature is safer.
