Yes, you missed the part that said that the boiler would start at the setting on the face of the controller when the weather got warmer than 65F.

That setting is the MINIMUM temperature setting, not the max; the reset goes UP from that setting. This means that if you set the controller to 140F, the water would be heated to 140F to start and then go up from there as the weather got colder and dropped from 65F. As your Ergomax controller would already have called for a 150F minimum, the two leads that are attached to the same two terminals of the ErgoMax aquastat as the reset would mean nothing until the outdoors was cold enough to make the reset move the minimum temperature of the water that is controlled by the ErgoMax thermostat up over 150F.

Yes, the other sensor goes into the boiler outlet temperature. It could attach to the tank inside the insulation or in a well on the tank outlet piping.
