In most areas, adding a backflow preventer is involved with the local water supply. If this is about a well system, then that isn't involved.

Operationally, the backflow overcomes at least 40 psig, so vertical wouldn't be a pressure difference problem; but I'd call Honeywell to find out, as the drawings for the device show it only in the horizontal position.
