Boiler manufacturers voluntarily assisted the Department of Energy to test boilers in a consistent manner by taking efficiency and temperature readings during factory testing procedures. From these readings the Annual Fuel Utilization Efficiency (AFUE) is published to compare residential boiler operation.

It costs manufacturers to expose their engineering by the use of the test, so some won't spend the money. An IBR rating is a sign that manufacturers want to be forthright about their efficiencies.
