expansion tank and pressure reducing valve - avoiding the same problems

About 10 years ago, the boiler people installed an Amptrol hot water tank connected to my oil burner. The Amptrol tank burst about two years later. The same happened to my neighbor. They put another Amptrol in and that is beginning to burst too. I suspect they burst because of no expansion tank installed at the Amtrol boiler.

I'm getting rid of my boiler people and my Amtrol tank. I'd like to install my new Takagi TK-D20 instant hot water maker this week (water is preset at 122 F), but have a few questions:

(1) is there a need for an expansion tank? If so, where to install it?

(2) is there a need for pressure reducing valve. The city pressure is about 40 psi at the main control valve of the house.
