On all the corners. Looks like the radiator was built for one and two pipe systems. Thanks for clarifying...it should have been obvious. There are a few smaller nuts on tie rods as well.
Turns out the radiator magically fixed itself. The only thing we did change was the pitch because contractors had levelled the radiator when they put in a new floor two years ago. I'm surprised it took this long to affect that radiator, but I'm just glad it works at this point...
