If you have access to a digital camera or scanner, e-mailing a copy is possible. A modern flame retention burner may be an option. If your burner motor has 1725RPM on it's nameplate it is probably an older type. Look for a burner brand and model and the speed. A new burner could get you close to a new boiler in efficiency at $600 versus $3,000 or so.
