Piping steam boiler

Our contractor has just replaced our old steam boiler with with a new one. Questions about the header piping:
1. How important is it for the take-offs to come off from the header vertically? One take-off is vertical, but the other connects at a 45 degree angle. Will this cause unequal distribution.
2. What diameter should the take-offs be? The header is 4 inch, but the take-offs are 3 inch, connecting to 5 inch mains.
