I spoke with our water treatment guy the other day and he said that he spoke with someone with first hand info about the explosion and said that someone jumped out the low water cutoff. The operators left them unattended and came back, saw the guage glass was empty, and instead of shutting off the fuel source, decided to pump feedwater in, and boom. Sounds like a joke, but apparently this is how it went down.
