Your welds are actually failing or failing radiography ? Are these pipe to pipe welds or vessel nozzle welds - hard to tell from your post
What is the base metal ?

: I'm a R-stamp holder struggling with a vessel that has had H2S in it. My welds are failing at the rate of 4 out of 5. I run a 5P root and a 7018 cap. Some welds I've rolled out and still have problems. What am I doing wrong?
