When you mention "the front of the shell overheated" do you mean the underneath of the shell or do you mean the tube sheet overheated?
Give me a little more information... Is this a rivited boiler? What is the manufacturer's name? Is this a true HRT.. most of the them were two pass, or is this boiler a fire-tube firebox type with water legs on all sides.
