I am not so sure starvation was the cause of the tube failure, particularly since it was only one tube. How old is the waterwall tube? I fish-mouth type opening indicates sudden failure. Send the tube out for analysis. A good company is Thielsch Engineering in Cranston RI. They do a great job.
