There could be many reasons why the weld is failing. First, who is doing the welding? It must be done by a qualified welder under the direction of a company that is National Board certified ("R" Stamp). Make sure the proper weld procedure is being used. Depending on the size of the nipple, it may be under stress,vibration,etc. It may be necessary to completely remove all existing weld metal, re-prep the opening and start from scratch. The problem may be at the root of the weld.
