welding failure of the feed nipple entering the drum

ours is a water tube boiler. we are frequently finding one of the nipples joined to the drum is failing at the point of
welding. this failure occurs every two years or so. we have to reweld the nipple. this particular problem
is not periodic and is rather random. suddenly a crack will appear and it starts leaking steam.

what could be the reason for failure of only this welding? and why not any other nipple, such as gauge glass
nipple or instrument connection nipple or soot blower nipple or air vent nipple.

some body try to explain.
